Please bring these items with you on your first visit to our office:
Identification
Please bring identification for your child and proof of legal guardianship. We accept birth certificates, passports, and other government documents.
Insurance card
Visits can get expensive. Please bring proof of your child’s insurance if they have one.
Medical history
If you don’t have your child’s medical records, please ask your previous medical office to send your child’s medical history to us.
Diapers (if needed)
Office visits can be stressful to an infant child, so expect the unexpected.
Oh, and don’t forget to bring your child.
